POLST AND DNR FORMS
While Advance Directives outline general preferences, POLST (Physician Orders for Life-Sustaining Treatment) and DNR (Do Not Resuscitate) forms are medical orders — meant to be honored in real-time by healthcare professionals, including EMTs and emergency teams.
These forms are especially important when:
- You have a serious illness or are near the end of life
- You want to limit or decline certain emergency treatments (like CPR, intubation, or hospitalization)
- You want your care plan to be immediately actionable and medically recognized
POLST forms are not for everyone. They’re designed for people facing specific health conditions, and they must be signed by a healthcare provider.
A DNR alone won’t cover everything — but it may be one important part of your plan.
